Metallosis! Hardware failure, especially in hip arthroplasties, can be a result of the following pathologies: 1. Simple aseptic loosening – diagnosed with progressive more than 2mm peri-hardware or peri-cement lucency and not associated to infection. Lucency about hardware is usually symmetric. 2. Infection – peri-hardware lucency greater than 2mm, but unlike loosening, usually asymmetric, focal and destructive. 3. Particle disease – previously discussed and related to debris released by hardware which in turn incites osteolysis of the surrounding bone. 4. Metallosis – also known as hardware pseudotumor and usually co-existing with particle disease and osteolysis. In imaging, metallosis is seen as a soft tissue pseudo mass created by metal debris. This mass usually is dense on radiographs and CT, due to the presence of metal.
