In this radiograph, there is air within the soft tissues and bone of the lateral forefoot, which was confirmed with CT scan. In this diabetic patient, emphysematous osteomyelitis was suggested. This is a serious (38% mortality rate) and rare condition. But not all air within the bone is related to osteomyelitis. There are other culprits; including osteonecrosis, open wound/fracture, post biopsy, lymphangiomatosis and some bone tumors. This particular case was early necrotizing fasciitis with emphysematous osteomyelitis.
